In this blog, we will look at a case in which a high-temperature and high-pressure heat exchanger burst along the welds during operation at an oil and gas worksite. This incident had the potential to cause significant damage to both the complex and its employees.
Leveraging our team's extensive expertise and experience in analysing various devices and components, including high-temperature/pressure reactors, valves, nozzles, pipes, tanks, heat exchangers, boilers, and more, we conducted an investigation at the worksite and analysed the case at the TÜV SÜD laboratory, which is equipped with advanced technology.
Upon investigation, we discovered that the device's welds had burst due to caustic embrittlement of the steel. This issue was attributed to inappropriate post-welding heat treatment and water quality control, rather than operational causes.
After identifying the root cause of the failure, the plant operator implemented appropriate remedial actions and received suggestions on improving maintenance to prevent a recurrence of the issue in the future.
